For some reason, I have this urge to ask my lj audience to give me advice. I don't know why. And I also don't know what advice I want to ask for.

So I figured, why not ask for advice on that one first?

I do lots of polls, I think they're great fun. My only advice is to make sure you ask questions that a) people will feel comfortable answering and that 2) you'll be comfortable reading the answers to.

Oh, and beware of using radio buttons instead of checkboxes...if your options aren't absolutely mutually exclusive, let people check more than one.
